APPLYING TO REGISTER WITH THE PRACTICE
When you register with the practice you will be asked to fill out a registration form or hand in your completed NHS Medical Card. We will decide if we will accept you onto our list. You will be asked to make a new patient appointment with a Practice Nurse, and to fill in a health and lifestyle questionnaire.

You will not be registered with any one particular doctor but you can request to see a particular doctor if you so wish, although this may mean a longer wait for an appointment or advice.
